Requisitions and Labeling
Requisitions
All patient specimens must be accompanied by a Covenant HealthCare Laboratory Test Requisition. All fields on the requisition must be completed. The patient names on the requisition and specimen container must match exactly.
Labeling of specimens
Unlabeled specimens will not be processed.
Specimen containers must be labeled with patient name, identification number (DOB, medical record, etc.), date and time of collection, and initials of collector.
